Myocardial Infarct with Rupture

View of posterior aspect of heart. The prominent band of fat extending from the base of the heart to the apex marks the location of the IV septum and the posterior descending coronary a.. The lateral and posterior LV walls are above the fat band and the RV wall below.
The arrow points to a subepicardial hematoma, due to a hemorrhagic transmural ruptured infarct. The actual rupture of the distended epicardium is not visible in this view, but the arrow points to the rupture site.
