Calcification of Aortic Valve with Stenosis

This is a surgically excised aortic valve, replaced by a prosthesis.
View is from above. Note the severe stenosis due to fusion of the 3 commissures, which are obliterated, marked fibrous valve thickening, and calcification, with irregular calcific excrescences jutting into the orifice (arrow).
Fusion of the commissures indicates that the etiology of this type of aortic stenosis is rheumatic fever, in contrast to calcific aortic stenosis with intact commissures and discrete calcified valves in the aged of unknown etiology.
