Fibrosis and Calcification of Mitral Valve

This is an excised stenotic mitral valve, which was replaced by a prosthesis.
This is a classic example of mitral stenosis due to rheumatic fever.
The valve leaflets, anterior above, and posterior leaflet below, are markedly thickened by fibrosis and focal calcification. The commissures (2 arrows) are fused forming a stenotic orifice, which has been characterized as a "fish mouth" or "button hole" deformity.
