• The glands contain secretion and are coiled. • The glandular epithelium is composed of glandular cells with eosinophilic cytoplasm. • The intervening stroma is composed of predecidualized cells. • The stromal cells have round nuclei and eosinophilic cytoplasm.

General Gross Description

tan, soft, measuring up to 4 mm in thickness
General Microscopic Description

KEY feature is the presence of glandular secretions
glands become tortuous with progression of secretions from basal vacuoles (d 17-18), supranuclear vacuoles (d 19-20), active intraluminal secretion (d 21-22)
spiral arteries become prominent (d 21-22) and develop predecidual stromal cuffing (d23-24)
stromal edema develops (d21-22) with subcapsular (d 25) and full thickness predecidual change (d 26)
leukocytic infiltrate pronounced by d 27-8.
Clinical Correlation

normal in the luteal phase of the menstrual cycle
maturation of the epithelium previously treated with estrogen by the influence of progesterone
menstruation begins with the loss of progesterone and vasoconstriction of spiral arteries
